RequireJS for modular JavaScript code
Nowadays, our server-side code looks reasonably well. We use TDD together with sturdy domain-driven code sprinkled with one more level of indirection to solve just that tricky problem.
And then there's our client-side code. With .js files spanning thousands of rows, containing top-namespaced functions called from everyone-and-her-mother making change a bitch. And you the bitch of change.
